The Houston/Harris County Integrated Healthcare Workgroup for Serious and Chronic Illness, launched in 2011, facilitated formal discussion among local chronic disease affiliate organizations in order to create an action plan for addressing the mental health needs of their constituencies as part of an integrated approach to care. MHA will continue to work with local chronic disease affiliate organizations to implement the action plan, which includes training for health organization staff, training for mental health professionals and caregiver education.
